What Is Digital Motion X-Ray?
Digital Motion X-Ray — also known as DMX or fluoroscopic motion X-ray — is a real-time, dynamic imaging technology that captures continuous X-ray video of your spine and joints as you move through a full range of motion.
Unlike a standard X-ray, which produces a single static image of your skeletal structure at one moment in time, DMX records what happens to your cervical and lumbar spine during actual movement — flexion, extension, lateral bending, and rotation. This allows our physicians to observe spinal alignment, facet joint function, and — most critically — ligamentous integrity in a way that no static imaging technology can replicate.
Why Movement Changes Everything in Injury Diagnosis
Ligaments — the connective tissue that holds your vertebrae in proper alignment — do not reveal their full condition on standard MRI or X-ray. When a ligament is stretched, torn, or weakened by the forces of a collision, the instability it creates only becomes visible when the spine is loaded and in motion.
A static MRI may show discs and bones appearing entirely normal even when the ligaments holding those structures together have been significantly compromised. DMX captures that instability in motion, providing objective, measurable documentation of injury that static imaging cannot.
How DMX Compares to Standard Imaging
Standard X-ray: Single static image. Shows bones only. Captures no movement. Cannot evaluate ligament function or detect instability. A "normal" result does not rule out significant soft tissue or ligamentous injury.
Standard MRI: Excellent for disc pathology and nerve impingement. Static by nature — images taken at rest with no load on the spine. Ligamentous injuries that only manifest during movement are routinely missed.
Digital Motion X-Ray: Continuous real-time imaging. Captures the spine throughout a full range of motion. Directly evaluates ligamentous integrity under functional load. Provides objective, frame-by-frame documentation of injury that complements — rather than replaces — MRI findings.
What Injuries Does DMX Detect?
DMX is particularly valuable for identifying injuries commonly sustained in motor vehicle accidents that are frequently underdiagnosed with conventional imaging, including:
- Cervical ligamentous instability — among the most common missed injuries following whiplash-type trauma
- Alar and transverse ligament injury affecting upper cervical stability
- Segmental hypermobility and abnormal intersegmental motion patterns
- Lumbar instability exacerbated or revealed by traumatic loading
- Facet joint dysfunction visible only under motion loading conditions
- Post-traumatic loss of cervical lordosis or kyphotic deformity under load
- Soft tissue injuries producing measurable biomechanical changes in spinal motion
Who Should Have a DMX Evaluation?
DMX is recommended for accident patients who:
- Have persistent neck or back pain following a motor vehicle accident
- Have received a "normal" MRI or X-ray result despite ongoing symptoms
- Experience pain, instability, or neurological symptoms with movement
- Have headaches, dizziness, or visual changes consistent with upper cervical involvement
- Require comprehensive imaging documentation of their injuries
